SAUCONY EVENT WAIVER AND RELEASE

In consideration of the opportunity to participate in the Saucony demo run event (the "Event") on Day / Month / Year , and other events and activities related to my participation (collectively, the "Event"), I , the undersigned, acknowledge and agree that:

1. Transportation to, from, or during the Event, participation in the Event and attendance at the Event involves inherent risks and dangers of accidents, personal and bodily injury (including death) and property loss or damage. These may result from my own actions or inactions, as well as the actions or inactions of others (including but not limited to participants, volunteers, spectators, coaches, event officials, event monitors and/or producers of the event), the rules of play, lack of hydration, the condition of the facilities and equipment, weather, and/or other natural conditions. Further, there may be other risks not known to me and not reasonably foreseeable at this time. I have considered the nature and extent of the risks involved and I voluntarily choose to assume all such risks, both known and unknown, even those risks that result from the negligence of the Released Parties (defined below) or others and assume full responsibility for my participation in the Event. I consent to treatment in the event of an emergency or other incident in which, in the reasonable judgment of the on-site personnel, I require medical care.
2. I certify that I am physically fit, have sufficiently trained for participation in this event and have not been advised otherwise by a qualified medical professional.
3. I acknowledge that this Event Waiver and Release Form will be used by Wolverine World Wide, Inc., the Event sponsors, Event co-sponsors, the Event volunteers, and each of their respective subsidiaries, affiliates, officers, directors, shareholders, employees, agents and representatives, successors, assigns, and insurers (collectively, the "Released Parties") and it will govern my actions and responsibilities at said Event.
4. In consideration of the opportunity to participate in the Event, I, for myself and on behalf of my heirs, estate, insurers, successors and assigns, hereby: (A) waive, release, and discharge the Released Parties with respect to any and all claims or causes of action I may have for damages for personal or bodily injury, disability, death, loss or damage to person or property, whether arising from the negligence of any or all of the Released Parties or otherwise, to the fullest extent permitted by law; (B) indemnify, hold harmless, and covenant not to sue the Released Parties from any and all liabilities or claims related to or arising from my travel to or participation in the Event or my actions in connection with the same.
5. I understand that the Event may be photographed, videotaped, or otherwise recorded, and I hereby expressly grant the Released Parties the perpetual, global right to use my name, likeness, voice, quotes, statements and/or any personal information I have provided (in any form and without regard to distortions of character, form or color, or any other alteration), as recorded in association with the Event, for advertising and promotion of the Released Parties and their products and services, in any form or media, without any other compensation to me, or to any third party acting on my behalf or for my benefit, other than being allowed to participate in and/or attend the Event. I waive any right to inspect and/or approve any such use of my name likeness, voice, quotes, statements and/or personal information.
6. I have read this Event Waiver and Release, fully understand its terms and understand that I am relinquishing substantial rights by signing it. I sign this Event Waiver and Release freely and voluntarily, without any inducement or coercion.
